We explore inflation driven by the Starobinsky-like R + R² term using a manifestly gauge-independent approach to cosmological perturbation theory. Working directly with the metric degrees of freedom in quadratic gravity, we derive the evolution equations for gauge-invariant scalar perturbations without recourse to an auxiliary scalar field or conformal transformation. The Mukhanov-Sasaki variable is constructed from first principles and shown to satisfy the standard equation of motion, confirming consistency with the scalar-tensor formulation. We compute the scalar spectral index and tensor-to-scalar ratio and demonstrate agreement with Planck constraints. Additionally, we analyze the evolution of additional gauge-invariant variables that arise naturally in the higher-derivative theory, clarifying their role during and after inflation. Our results provide a self-contained framework for studying perturbations in quadratic gravity that is entirely independent of the gauge choice and offers new perspectives on the physical degrees of freedom during inflation.
